Note: while Creative Edge is still accepting new clients, we will be located in Mexico from August through February every year. During those months all sessions will be conducted either online or by phone. rather than in person.

New Client Intake Form
To apply to become a new client, or, assuming no immediate openings, to have your name placed on our waiting list, send us an email with the following information:
First and Last Name
Age
Gender
Marital status
Children
Career Status
Circumstances/issues that lead you to seek counselling
Anything else about your personal history that you think might be relevant or that you would want us to know about
Goals for counselling
Previous experience with counselling
Anti-depression medication, if anyHow you found out about Creative Edge
What appealed to you about the way we describe our services
Availability for sessions during regular office hours
Telephone numbers and the best times to call.
Home Address and Postal Code
Interest in a referral if our assessment indicates that another service would be more appropriate to your needs, or if there aren't likely to be openings in the immediate future
Remember that our approach is holistic. Issues of body, mind, heart and spirit are all relevant.
When in doubt about how much to write, err on the side of more information rather than less, but keep it to a maximum of four pages.

Sending your application will trigger an automatic reply indicating that your email has been received. You will hear from us again once we have been able to assess your application. Generally we can get back to you within a week to ten days..
If there seems to be a good match between what you need and what we do, and
if there is an opening, or the likelihood of an opening in the near future, we will
contact you with our current rates and availability and either set up a consultation or
offer you a place on our waiting list.
If we are aware of services that are either a better match for your needs or offer a
shorter waiting time, we will contact you with suggestions for alternate services that you
might want to try.
